A. All requests for a wellness check shall be submitted to the Department of Social Services. Any department of the Tribe that incorrectly receives a request for a wellness check shall forward the request for a wellness check to the Department of Social Services.

B. Requests for a wellness check shall include supporting evidence, such as pictures or statements, that explains or provides proof as to the need of a wellness check and the type of abuse suspected to be or has been occurring. (Res. 2023-59)
