The member’s signature on a request for per capita distribution forms must be verified in one (1) of two (2) of the following ways:

A. A designated representative at the Tribal Office may verify the signature of a member on a request for per capita distribution form.

1. The authorized representative must see the individual sign the request for per capita distribution form.

2. The authorized representative must check a government-issued photo identification of the individual signing the request for per capita distribution form.

B. The member may have his or her signature on the request for per capita distribution form notarized by a notary public. A notary public is an individual who holds an active notary public commission from a state government. (Res. 2014-38; Res. 2013-15)
